A photo of your card on your phone is enough. We'll handle the rest at check-in.

Plan for 10 minutes of paperwork buffer for first visits — and a quick office tour for the kids.
Bring with you
A short checklist for the day-of.
- Insurance card (or a phone photo)
- Photo ID for the parent or guardian
- Names of any current medications
